Your campaign writes itself into the world
Build as much of your world up front as you like. Saga20 gives you the entries, the custom sections, the maps and the visibility controls to do it properly, and plenty of DMs spend happy weeks there before session one.
Then you play, and the campaign starts generating its own history. The party names the innkeeper and the name sticks.
